Terms of Service.

These Terms of Service govern your use of the PhysioVille website (physioville.ca) and the clinical services we provide at 3328 McCarthy Road in Ottawa. They cover appointment policies, payment terms, patient responsibilities, and standard legal provisions.

Our story

Built as a village, not just a clinic.

Healing and development rarely happen through one appointment, one discipline or one person. They happen when people feel supported.
That belief is behind PhysioVille. We bring adult rehabilitation and specialized pediatric physiotherapy together with a growing multidisciplinary team. Our goal is to make care feel connected, understandable and genuinely welcoming—from a child working toward a first step to an adult returning to work, sport or everyday confidence.

No. 01 Acceptance of these Terms

What you're agreeing to.

These Terms of Service ("Terms") form a binding agreement between you and PhysioVille ("we," "us," "our"). By visiting our website, booking an appointment, or receiving care at our clinic, you confirm that you have read, understood, and agree to be bound by these Terms.

If you don't agree with any part of these Terms, please don't use our website or services. If something is unclear, we'd rather answer your questions than have you accept something you don't understand — please contact us.

These Terms apply alongside our Privacy Policy, which describes how we handle your personal information and personal health information. Together, they govern your relationship with our clinic.

Where a patient is a minor (under 18), a parent or legal guardian agrees to these Terms on the child's behalf.

No. 02 About Our Services

Our website is not medical advice.

PhysioVille is a physiotherapy clinic in Ottawa's Hunt Club neighbourhood. Our clinical services are provided by a Registered Physiotherapist in Ontario, in accordance with the practice standards of the College of Physiotherapists of Ontario.

Website content is general information only

The information on our website — including descriptions of conditions we treat, treatment approaches, and general guidance — is provided for informational and educational purposes only. It is not personalized medical advice. Reading our website does not establish a physiotherapist-patient relationship.

A physiotherapist-patient relationship is established only when you (or your child, for pediatric patients) attend a formal assessment at our clinic and we agree to provide care.

Website content is not a substitute for professional care

Do not rely on our website content to diagnose, treat, or manage a medical condition. If you have a concern about your health or your child's development, please book an assessment — or speak with your family physician, pediatrician, or another qualified healthcare provider.

In an Emergency

Do not use our website, email, or booking system for emergencies.

If you are experiencing a medical emergency — including severe pain, difficulty breathing, sudden loss of consciousness, stroke symptoms, serious injury, or a mental health crisis — call 911 immediately or go to your nearest emergency department.

For an urgent but non-emergency health concern, contact Telehealth Ontario at 811 or your family physician.

No. 03 Booking & Appointments

How to book, and what to expect.

How to book

Appointments can be booked online through Jane App (linked from our website), by phone at 416 897 6119, or by email at physioville.huntclub@gmail.com.

When you book, we collect the information needed to schedule your appointment and prepare for your visit. See our Privacy Policy for details on how that information is handled.

New patient assessments

Initial physiotherapy assessments are longer than follow-up sessions to allow time for a full history, physical examination, and discussion of your treatment plan. Please arrive a few minutes early to complete any intake forms. For pediatric assessments, please allow extra time for your child to feel comfortable in the space.

Referrals

You do not need a physician's referral to receive physiotherapy in Ontario. However, some insurance providers require a referral for reimbursement — please check with your insurer if you plan to submit claims for reimbursement.

No. 04 Cancellations & Missed Appointments

Our appointment policy.

Appointment time is reserved specifically for you. When appointments are cancelled at short notice or missed, that time can't be offered to another patient who might have needed it.

We ask that you give us at least 24 hours' notice if you need to cancel or reschedule an appointment. This lets us offer the time to someone on our waitlist.

We understand things happen

Sudden illness, a family emergency, weather that closes roads — real life doesn't always give 24 hours' notice. We consider genuine extenuating circumstances on a case-by-case basis. If something has come up, contact us as early as you can and we'll work with you.

Late arrival

If you arrive late for your appointment, we'll do our best to fit in as much of the scheduled treatment as possible. However, we cannot extend the appointment past its scheduled end time, and the full session fee will apply.

Repeated missed appointments

We reserve the right to discontinue care in cases of repeated missed appointments without notice. Where this happens, we will make reasonable efforts to help you transition your care to another provider.

No. 05 Payment, Fees & Insurance

How billing works.

Payment is due at the time of service

Payment for physiotherapy services is due at the time of the appointment, unless we have made specific arrangements with you or your insurance provider in advance. We accept credit card, debit, and cash. Payment card transactions are processed by third-party payment processors — we do not store your full payment card details.

Extended health benefits

We may be able to submit claims directly to your extended health benefits provider on your behalf. Direct billing depends on your specific insurance company and plan. If direct billing isn't available for your plan, we'll provide you with a receipt so you can submit for reimbursement yourself.

Please note: you are ultimately responsible for payment for services received, whether or not your insurance reimburses. We recommend confirming your coverage with your insurer before beginning treatment.

Motor vehicle accident (MVA) claims

We work with motor vehicle insurance providers under Ontario's Statutory Accident Benefits Schedule (SABS). If you have been in a motor vehicle accident, please bring your claim information to your first appointment so we can coordinate billing.

WSIB claims

We work with the Workplace Safety and Insurance Board (WSIB) for work-related injuries. Please bring your WSIB claim number to your first appointment.

Fees for services outside standard care

Some services, such as detailed written reports, insurance form completion, or extended consultations outside a treatment session, may be subject to additional fees. We'll always tell you about these fees before providing the service.

Tax

Physiotherapy services provided by a Registered Physiotherapist are generally exempt from GST/HST in Canada. Non-clinical services (such as certain products or written reports) may be subject to applicable taxes.

No. 06 Your Responsibilities as a Patient

What we ask of you.

Good physiotherapy is a partnership. To get the best out of your care, and to help us treat everyone in our clinic well, we ask that you:

  • Provide complete and accurate information about your health history, symptoms, medications, and any pre-existing conditions.
  • Tell us promptly about any changes in your condition or symptoms between appointments.
  • Follow your treatment plan and home program to the best of your ability, and tell us honestly if something isn't working.
  • Attend appointments on time, prepared, and dressed appropriately for the treatment being provided (comfortable clothing that allows access to the area being treated).
  • Give us reasonable notice if you need to cancel or reschedule (see Section 04).
  • Treat our staff and other patients with respect. We reserve the right to discontinue care in cases of harassment, threats, or abusive behaviour toward staff or other patients.
  • For pediatric patients: parents or guardians agree to stay on the premises during appointments unless we have specifically discussed otherwise, and to participate in the care plan as we discuss.
  • Follow any clinic policies posted or communicated to you (such as infection-control measures, footwear requirements, or scent-free environment guidelines).

No. 07 Website Use & Intellectual Property

Ownership and acceptable use of our website.

Our content is our property

All content on our website — including text, photographs, illustrations, icons, the PhysioVille logo and name, page layouts, and design elements — is owned by PhysioVille or licensed to us, and is protected by Canadian and international copyright and trademark laws.

You may view, print, and share links to our website for your own personal, non-commercial use. You may not:

  • Copy, reproduce, republish, or distribute our content for commercial purposes without our written permission
  • Modify or create derivative works based on our content
  • Use our logo, name, or branding in a way that suggests affiliation, sponsorship, or endorsement
  • Remove or alter any copyright notices or attributions

If you'd like to use our content in a way not covered above (for example, quoting us in an article or referencing us professionally), please contact us to discuss.

Acceptable use

You agree not to:

  • Use the website in any way that violates any applicable law or regulation
  • Attempt to gain unauthorized access to any part of the website, our systems, or our patient records

Services and websites we don't control.

Booking through Jane App

Our online booking is provided through Jane App, a third-party clinic management platform. When you book through Jane App, your interaction with that platform is also governed by Jane App's own Terms of Service and Privacy Policy, which are available at jane.app.

Links to other websites

Our website may contain links to third-party websites, such as insurance providers, professional organizations, or educational resources. We provide these links for your convenience only. Linking to a website does not mean we endorse it, its content, or its practices.

We are not responsible for the content, privacy practices, or terms of use of third-party websites. If you follow a link off our site, you do so at your own discretion and should review the terms and privacy policies of the site you visit.

No. 09 Disclaimers & Limitation of Liability

The legal limits of what we can promise.

Website provided "as is"

Our website is provided on an "as is" and "as available" basis. While we take reasonable care to keep the site accurate, current, and functional, we do not warrant that:

  • The website will be uninterrupted, error-free, or free of viruses or malicious code
  • All content is accurate, complete, or up to date at every moment
  • The website will meet your particular requirements or expectations

Clinical services

Our physiotherapist provides clinical services with the reasonable skill, care, and diligence expected of a Registered Physiotherapist in Ontario. However, physiotherapy outcomes depend on many factors — including the nature of your condition, your engagement with the treatment plan, other aspects of your health, and factors outside our control. We cannot guarantee specific outcomes or timelines.

Limitation of liability

To the maximum extent permitted by law, PhysioVille and our staff shall not be liable for:

  • Any indirect, incidental, consequential, special, or punitive damages arising out of your use of our website or services
  • Loss of profits, revenue, data, or business opportunity
  • Damages resulting from your reliance on general information provided on our website
  • Damages caused by third-party services, links, or content

Nothing in these Terms excludes or limits our liability for death, personal injury caused by our negligence, fraud, fraudulent misrepresentation, or any liability that cannot be excluded or limited under applicable law.

Nothing here limits your rights as a patient

These limitations apply to general legal claims. They do not affect your rights to file a complaint with the College of Physiotherapists of Ontario about the professional conduct of your physiotherapist, or to access other regulatory or consumer-protection remedies available to you under Ontario law.

No. 10 Indemnification

Your obligations if things go wrong through your actions.

You agree to indemnify, defend, and hold harmless PhysioVille and our staff from any claims, damages, losses, liabilities, and expenses (including reasonable legal fees) that arise from:

  • Your breach of these Terms
  • Your misuse of our website or services
  • Your violation of any applicable law or the rights of a third party
  • Any false, misleading, or inaccurate information you provide to us

This obligation survives the termination of your relationship with our clinic.

No. 11 Governing Law & Jurisdiction

The law that applies.

These Terms are governed by the laws of the Province of Ontario and the applicable laws of Canada, without regard to conflict-of-laws principles.

Any dispute arising out of or relating to these Terms, our website, or our services shall be subject to the exclusive jurisdiction of the courts of Ontario, located in Ottawa. By using our website or services, you consent to the personal jurisdiction of those courts.

If any provision of these Terms is found to be invalid or unenforceable, the remaining provisions will continue in full force and effect. The invalid or unenforceable provision will be modified only to the minimum extent necessary to make it valid or enforceable.

How we handle updates.

We may update these Terms from time to time to reflect changes in our practices, our services, or applicable law. When we make material changes, we will update the "Last Updated" date at the top of this page and, where appropriate, notify current patients.

Your continued use of our website or services after an update means you accept the updated Terms. If you don't agree with an update, please stop using our services and contact us so we can discuss any concerns.
