Everything you need before your first visit.
Booking, your first appointment, fees, insurance and forms, all on one page. Coming in for yourself or your child, we want your first visit to feel easy.

Your first visit, step by step.
Before we talk about roles, this is what we believe about clinical work. If any of the below makes you uncomfortable, this probably isn't the right clinic for you, and that's a useful thing for both of us to know early.
Bring what helps us understand the whole picture.
A little preparation can help your first appointment feel smoother.
If you do not have every item, you can still contact the clinic for guidance before your visit.
Wear clothing that allows the relevant area and movement to be assessed.
Bring reports, imaging summaries, referrals, or care-team notes when they are relevant and available.
Be ready to describe the concern, changes over time, current supports, and activities affected.
Check your individual plan for reimbursement requirements before the appointment.
Download, fill, bring. Or do it when you arrive.
Completing forms before you arrive saves 10–15 minutes on first-visit paperwork, and lets us start treatment sooner. All forms are PDF; print or fill digitally.
Adult Intake Form
Medical history, current symptoms, goals for treatment. 4 pages, 10 minutes.
Pediatric Intake Form
Developmental history, milestones, parent concerns. 5 pages, 12 minutes.
Consent to Treatment & Privacy
Required for all new patients. Covers consent to assessment and treatment, plus privacy acknowledgment.
Insurance & Direct Billing Authorization
Required to submit claims directly to your extended health insurer on your behalf.
We bill your insurance directly.
PhysioVille direct-bills most major insurers wherever possible. WSIB and motor vehicle accident claims can also be handled in-house.
If your plan does not support direct billing, you can pay per visit and receive a receipt for reimbursement. Contact the clinic to confirm your specific coverage.

Do I need a referral?
No. Physiotherapists in Ontario can be seen directly. Some insurance plans ask for a doctor's note before they reimburse, so check your plan.
What should I wear?
Comfortable clothes you can move in. For knee or hip concerns, shorts are helpful. For shoulders or the neck, a tank top works well.
How long until I feel better?
It depends on your condition. At your assessment we'll give you a realistic timeline and the number of visits we expect.
Can siblings come to my child's appointment?
Yes. Our pediatric space is family-friendly. Let us know when you book so we can plan the session.

Do I get anything for referring?
No, and that’s on purpose. We don’t believe health care should be traded for gift cards, and professional rules for physiotherapists in Ontario restrict benefits tied to referrals. What you get is a friend who’s finally getting help.
Do you collect my friend’s contact details?
Never. The message builder runs entirely on your device. Nothing you type is sent to us. You send the message yourself, and your friend decides whether to get in touch.
Will you tell my friend about my treatment?
No. Your health information is confidential under Ontario law. We won’t confirm or discuss your care with anyone, including the people you refer.
Can I refer a child or an older parent?
Yes. We see infants through older adults. For a child, send the message to their parent or guardian. They’ll book a pediatric assessment, where the family stays in the room the whole time.
What if I’m not sure physio is right for them?
Tell them to call. A two-minute conversation is usually enough for us to say “come in” or “start with your doctor first.” We’d rather point them in the right direction than book an appointment that won’t help.
